Infinite Power Gym Waiver Form 

Participants are voluntarily participating in the personalized health and fitness program offered through The Infinite Power Gym.

Participant recognizes that the program may involve strenuous physical activity including, but not limited to, muscle strength and endurance training, cardiovascular conditioning and training, and other various fitness activities.

Participant hereby affirms that they are in good physical condition and do not suffer from any known disability or condition which would prevent or limit their participation in exercising.

Participant acknowledges that enrollment and subsequent participation is purely voluntary and in no way mandated by Infinite Power Gym or its affiliates.” 

“In consideration of participation at the gym, Participant hereby release Infinite Power Gym and its affiliates from any claims, demands, and causes of action as a result of voluntary participation."

“Participant fully understands that injuries could occur as a result of exercising and hereby release Infinite Power Gym and its affiliates from any liability now or in the future for conditions that may occur. These conditions may include, but limited to, heart attacks, muscle strains, muscle pulls, muscle tears, broken bones, shin splints, heat prostration, injuries to knees, injuries to back, injuries to foot, or any other illness or soreness that may incur, including death.”

Participants HEREBY AFFIRM HAVING READ AND FULLY UNDERSTAND THE ABOVE STATEMENTS.
